Have usual residence in Extremadura

The Administration will collect the following data from the applicant and the person holding their legal representation, unless expressly stated otherwise, in which case they must mark a cross in the corresponding boxes in the annex and provide the documentation indicated below: 

 
a) Applicant:

• National Identity Document/NIE/Passport
• Medical, psychological, psychopedagogical, diagnostic testing, etc., reports that support health conditions that may lead to disability or worsening of these conditions.
• Self-assessment of Personal Income Tax for the last fiscal year, the deadline for filing of which has ended on the date of application submission.
• Certificates issued by the General Treasury of Social Security, MUFACE, MUGEJU, or another equivalent entity stating the affiliation, and, where applicable, the annual and monthly pension amounts for the month prior to the application submission registration date, expressly indicating the number of annual payments.
• For unemployed applicants, a jobseeker's card and, where applicable, a certificate issued by the SEPE (Spanish Employment Service) certifying the unemployment benefit received in the month prior to the date of registration for submitting this application, indicating its duration.
• Large family certificate, where applicable.
• In the case of transferring a file from another region, a copy of the document certifying the recognition and classification of the degree of disability, as well as the resolution of the degree and level of dependency, where applicable.
• Certificate of residence in Extremadura (no more than 5 days prior to the application date). For children under 5 years of age, residence in Extremadura must be certified by their guardian.
•  Academic degrees.
• Certificate proving victim status, where applicable. 

b) The person holding legal or voluntary representation:
