  • Submission Period

    Permanente.

  • Purpose

    Recognizing the status of dependency in order to be entitled to the benefits and services recognized in Law 39/2006, of December 14, on the Promotion of Personal Autonomy and Care for People in Situations of Dependency, and thus be able to meet the needs of those people who, due to being in a situation of special vulnerability, require support to carry out essential activities of daily living, achieve greater personal autonomy, and be able to fully exercise their citizenship rights.

    People who may be affected by some degree of dependency and wish to be assessed

  • Requirements

    1. Be in a situation of dependency in any of the established degrees.
    2. For children under 3 years of age, the provisions of the thirteenth additional provision of Law 39/2006, of December 14, shall apply.
    3. Reside in Spanish territory for five years, two of which must be immediately prior to the application submission date.
    4. For children under 5 years of age, this requirement applies to the person exercising their custody. When custody is shared, the residency period will be required for one of the persons exercising custody.
    5. Persons who do not have Spanish nationality shall be governed by the provisions of Organic Law 4/2000, of January 11. In the case of minors who do not have Spanish nationality, current laws on minors, both at the state and regional levels, as well as international treaties, shall be taken into account.
    6. In the case of returning emigrants who do not meet the residency requirement, they may request access to assistance benefits with the same content and scope as those established for dependent persons, provided they can prove their status.
    7. Reside in any municipality in Extremadura at the date of application submission.

  • Required Documentation

    Application (Annex I) which is mandatory.

    Optional documentation:

    • If you do not authorize the consultation, Report from the primary care professional on the applicant's health, in accordance with the standardized models, which, depending on the applicant's age, are included in Annex II, which explains, from a health perspective, the health conditions and, where applicable, the support products that have been prescribed.
    •  Optionally, a social report issued by a person with a degree in Social Work from the basic social care services or primary care health teams in accordance with the standardized model included in Annex IV.
    •  In the case of voluntary representation by a de facto guardian, Annex III, and in the case of legal representation, a court ruling certifying it.

    In addition to the above, the Administration will automatically collect the following data from the applicant, the person holding the representation, the members of the family unit or the caregiver, unless the aforementioned persons expressly state otherwise, in which case they must mark the corresponding boxes in Annex I with a cross and provide the documentation indicated below:

    • Applicant:
      • National Identity Document/NIE/Passport.
      • Certificate of registration proving residence in the Autonomous Community of Extremadura (maximum 5 days prior to the date of the application). In the case of children under five years of age, residence in the Autonomous Community must be accredited by the person who exercises his/her guardianship and custody.
      • In the case of transferring the file to another Autonomous Community, an updated registration certificate proving residence.
      • Resolution recognizing the degree of disability.
      • Certificate proving marital status.
      • If applicable, official certificate proving the status of common-law couples.
      • In the event that the applicant is a minor and does not have a National Identity Document (DNI), in accordance with Law 20/2011, of July 21, on the Civil Registry, a sheet or extract issued by the Civil Registry containing the personal data of the applicant's life.
      • Personal Income Tax self-assessment for the last fiscal year whose filing deadline ended on the date of application submission. For subsequent years, you must submit, before September 30 of each year, a Personal Income Tax self-assessment for the last fiscal year whose filing deadline ended before that month.
      • Self-assessment of the Wealth Tax for the last fiscal year whose submission deadline has ended on the date of submission of the application or, failing that, a list of all my assets valued in accordance with Law 19/1991, of June 6, on Wealth Tax. For subsequent years, you must submit, before September 30 of each year, a self-assessment of the Personal Income Tax for the last fiscal year whose submission deadline has expired has ended before the aforementioned month or, failing that, a list of all my assets valued in accordance with Law 19/1991, of June 6, on Wealth Tax.
      •  Updated cadastral certification for all your properties, including your primary residence. For subsequent years, you must submit updated cadastral certificates for the aforementioned properties by September 30 of each year.
      • Certificates issued by the General Treasury of Social Security, MUFACE, MUGEJU or other equivalent entity stating the annual amount and the monthly amount of the pension for the month prior to the registration date of the presentation of the application must specifically indicate the number of annual payments. For subsequent years, you must submit an updated pension certificate by September 30 of each year.
    • The person who holds legal/voluntary representation.
      • National Identity Document/NIE/Passport
    • Members of the family unit / common-law partner (per person).
      • National Identity Document/NIE/Passport.
      • Certificate of residence in the Autonomous Community of Extremadura (no more than 5 days prior to the application date). For children under five years of age, residence in the Autonomous Community must be certified by their guardian.
      • Personal Income Tax self-assessment for the last fiscal year whose filing deadline ended on the date of application submission. For subsequent years, you must submit, before September 30 of each year, a Personal Income Tax self-assessment for the last fiscal year whose filing deadline ended before that month.
      • Self-assessment of the Wealth Tax for the last fiscal year whose submission deadline ended on the date of submission of the application or, failing that, a list of all my assets valued in accordance with Law 19/1991, of June 6, on Wealth Tax. For subsequent years, you must submit, before September 30 of each year, a self-assessment of the Personal Income Tax for the last fiscal year whose submission deadline ended before the aforementioned month or, failing that, a list of all my assets valued in accordance with Law 19/1991, of June 6, on Wealth Tax.
